Champion’s Mindset – Terms and Conditions for Sport Psychology Services

1. Introduction

1.1 These terms and conditions govern the agreement between Champion’s Mindset (referred to as “Service Provider” or “we” or “us”) and the individual receiving sport psychology services (referred to as “Client” or “you”).

1.2 By engaging in our sport psychology services, you agree to the terms and conditions set forth below. Please review them carefully before proceeding with the services.

2. Services

2.1 The Service Provider will provide sport psychology services to the best of their abilities, employing professional knowledge and experience.

2.2 The nature and scope of the services will be outlined and agreed upon by both parties prior to the commencement of services.

3. Payment and Cancellation

3.1 Fees for the sport psychology services will be as agreed upon between the Service Provider and the Client.

3.2 Payment for services is due in full at the time of service, unless alternative payment arrangements have been agreed upon in advance.

3.3 Cancellation or rescheduling of appointments must be communicated to the Service Provider at least 24 hours in advance. Failure to do so may result in a cancellation fee.

4. Confidentiality and Privacy

4.1 The Service Provider acknowledges and respects the confidential nature of the information shared by the Client during the course of the services.

4.2 All personal and sensitive information shared by the Client will be kept strictly confidential, unless required by law or with the Client’s informed consent.

4.3 The Service Provider will take reasonable measures to ensure the security and privacy of all client data and information collected during the provision of services.

5. Limitation of Liability

5.1 The Service Provider will exercise reasonable care and professional skill when providing sport psychology services. However, the Client acknowledges that the outcome of the services is not guaranteed.

5.2 The Client understands that they are solely responsible for their actions, decisions, and the results they achieve or fail to achieve as a result of the services provided by the Service Provider.

6. Governing Law and Jurisdiction

6.1 This agreement shall be governed by and construed in accordance with the laws of the United Kingdom. Any disputes arising from this agreement shall be subject to the exclusive jurisdiction of the courts of the United Kingdom.
